Eldercare Consultations
Pax Anima works with the elder and/or loved ones to develop and implement a carefully crafted and individualized approach to care needs that are focused on wellness dimensions and offer a structural support system.
The world of eldercare can be complex. This complexity is especially true in crisis situations where decisions often need to be made quickly and the availability of options may be limited. The goal of the Eldercare Consultation service at Pax Anima is to avoid such crisis situations by providing the individual elder and his/her loved ones with the skills and tools necessary to be pro-active in their own planning process by optimizing options, compassionately deal with life's changes and realistically maintain one's highest level of independence.
The Eldercare Consultation service at Pax Anima can help:
• determine what kind of assistance is needed
• develop an individualized plan of care specifically suited to one's own situation
• identify questions to ask in order to become an advocate for good care
• locate information about services in the community and caregiving resources
• gain a greater understanding and perspective on eldercare
• empower the individual to establish both short-term and long-term care goals
The Eldercare Consultation process begins with a complementary initial conference of approximately 30 minutes in length. This conference is conducted either in person or via telephone in order to gain a working knowledge of the individual's specific needs, discuss their care issues and determine the kind of assistance they are seeking. Information is gleaned through appropriate questioning in order to provide specific guidance.
The initial conference serves as the foundational building block in the development of an individualized Comprehensive Approach to Eldercare. Through additional in-person interactions, the additional building blocks of eldercare are constructed. These include the following:
Individual Assessment Needs Analysis - focused on a physical, spiritual and psycho-social approach in which a person's strengths and needs in these areas are reviewed
Living Space Safety Assessment
Eldercare Delivery System Assessment - focused on the navigation of elder living options.
Elder/Family coaching and support.
